Important Notices

  • Walk-ins are not permitted
  • All communications and submissions must be made by mail or uploaded through the web portal
  • Fax submissions are not accepted
  • CNAs and CMAs users should not select the 'Login" section. This section is designated for Nurse Aide Training Programs and Staff Development Facilities
  • Alliant will no longer be sending out Certification Cards

Digital Form Submission Required

To ensure efficient and accurate processing, the submission of handwritten forms is strongly discouraged. All applicants are required to complete and submit the online digital form and upload any supporting documentation electronically.

Document Attachment Guidelines

Please adhere to the following requirements when uploading files:
  • Maximum File Size: 20,000 KB (20 MB) per file
  • Permitted File Formats: PDF, TXT, DOC, DOCX, TIF, TIFF, EXCEL, JPG, JPEG, JPE (Note: PDF format is highly preferred whenever feasible)
  • Do not include any of the following characters in file names: / \ # < > ‘ “
  • If submitting multiple files, each document must be uploaded individually and assigned a unique file name
